About the research project:
This study proposes developing a stakeholder-informed roadmap to achieving universal health coverage (UHC) in India. While the COVID-19 pandemic has shown that health care is a crucially important investment for the economy, such an investment must be accompanied by a social compact that all Indians must have access to a similar quality of care without the risk of impoverishment. Thus, our main aim is to engage with key stakeholders across the healthcare landscape and those whose voices have rarely been heard: those who deliver health care and those who receive it.
The study expands on a district-level India UHC Service Coverage Index (UHC-SI) developed by IIM-Bangalore, based on the WHO-World Bank. We aim to explore what drives observed variation in district-level health status and importantly, what stakeholders see as different pathways to UHC in India. This proposed study is guided by two principles: One, that identifying paths to UHC must examine state and district-level variation in health systems strength and epidemiologic profiles. Two, identifying solutions requires the perspectives of a diverse range of stakeholders, premised on the role of people-centeredness and co-production in health systems.
Given the complex and fragmented architecture of India's health system, we propose a district-level primary qualitative research with stakeholders to examine the gaps, priorities and solutions to advance the path towards UHC.
The overarching aim of the study is to identify feasible pathways towards universal health coverage in India through generating evidence on district-level health system performance and priorities.
The study will use primary qualitative research with stakeholders, comprising focus group discussion and/or semi-structured in-depth interviews with five groups: community members/service users; community-level health actors; frontline health workers; health care providers; and administrators. The district is the unit of analysis, rather than states, to provide grounded insights that are insufficiently represented with respect to UHC.
